January 1, 2019 - Update - Verification of License RenewalThe Board discontinued issuing pocket cards for all license types. If you recently renewed your license or are an employer or insurance provider seeking to verify the status of a license renewal of an audiologist, hearing aid fitter, hearing aid dealer, speech-language pathologist, or aide verification must be obtained electronically from the eLicense Ohio license management system, License Look-up Page at the following link: _verifylicense. A printer friendly version option is available if you need documentation confirming the license status, e.g., individual's name, license number, issue date, expiration date, etc.
August 17, 2018 - Update on eLicense account registration e-mail and licensure renewalBeginning August 16, 2018, the Board will be sending an e-mail to licensees who have not yet registered for their account with the eLicense Ohio license management system. The e-mail will have our board name and logo at the top, instructions, your security code, and a link to eLicense Ohio to create your account. You must create an account in order to renew your license. The Board will mail a renewal letter to all licensees approximately one week before the license renewal period opens. Licensed audiologists, speech-language pathologists, dual licensees, and aides will receive their renewal letter in the mail the week of September 24th and can renew their license beginning October 2, 2018, with a license expiration date of midnight on December 31, 2018. Licensed hearing aid dealers and fitters will receive their renewal letter in the mail the week of October 29th and can renew their license beginning November 1, 2018, with a license expiration date of midnight on January 30, 2019.
July 5, 2018 - Update on Pocket Cards and eLicense Ohio New Account Registration - The Board discontinued issuing pocket cards for all license types. Licensees are required to display their wall certificate in a conspicuous place where the licensee practices. Verification of a license can be obtained from the eLicense Ohio license look-up page at the following link: _verifylicense. Licensees are responsible for knowing their license expiration date and should visit the license look-up page to confirm their license status and expiration date. In order to renew your license, you must first register for an account with eLicense Ohio to create your user name and password. If you have not yet registered for an eLicense Ohio account, the Board urges you to create your required new account now, prior to the license renewal period, by visiting the eLicense Ohio portal at: _communitieslogin.
(a) Each person licensed or issued a limited permit by the board, shall report to the board every change of residence address within 30 days after the change, giving both the old and new addresses. In addition to the residence address, the person may provide the board with an alternate address of record. If an alternate address is the person's address of record, he or she may request, in writing, that the residence address not be disclosed to the public. (b) Each person licensed or issued a limited permit by the board shall report to the board every change of name within 30 days after the change, giving both the old and new names. (c) This section refers to every person who holds an active, inactive, unexpired, suspended license or limited permit.
Within thirty (30) days after receipt of an application for a license, certificate, or limited permit, the board shall inform the applicant, in writing, whether the application is complete and accepted for filing or that it is deficient and what specific information or documentation is required to complete the application.
(a) To qualify for a limited permit, a person must have applied to the National Board for Certification in Occupational Therapy (NBCOT) to take the licensing examination within four (4) months of completing the education and fieldwork requirements for licensure or certification and request NBCOT provide their examination score report be forwarded to the Board. (1) Upon receipt from NBCOT, the applicant must forward to the Board a copy of the Authorization to Test (ATT) letter. (2) The applicant must provide documentation or other evidence to the Board, to prove that the applicant requested their examination score be sent from NBCOT to the Board, before a limited permit may be issued. (3) A limited permit shall only be valid for three (3) months from the date of issuance by the Board, upon receipt of a failing result, or two (2) weeks following the expiration of the applicants' eligibility to test period, whichever occurs first. (4) The limited permit holder must immediately notify the Board of the results of the examination. (5) The limited permit holder must provide to the Board the name, address and telephone number of his or her employer and the name and license number of his or her supervising occupational therapist (OT). Any change of employer or supervising OT must be provided to the Board, in writing, within five (5) days of the change. (b) A limited permit shall not be denied to an applicant that has completed the fingerprint,education, and examination requirements, yet is unable to provide transcripts due to the college or university's inability to make the transcripts available to the student or the Board in a timely manner. A limited permit issued pursuant to this section shall only be valid for three (3) months from the date of issuance by the Board. (c) The limited permit will be cancelled, and the fee forfeited, upon notification to the Board or the limited permit holder by the test administrator that the holder failed to pass the first examination. 2ff7e9595c
